Dear Parents and Students: 

Thank you for your support of Dove Schools.  Due to the limited space for the upcoming school year, we ask each parent/guardian to fill out this form completly.  This form is designed for currently enrolled students to update their information and determine whether the are returning for the 2022-2023 academic year to Dove Schools.  The due date to turn in this form is Monday, January, 31, 2022.  If the form is received unsigned, incomplete, or after the closing date, this may affect the student's enrollment status for the next school year.
